Gracefully Connected™ is a service provided by Gracefully Aging Network™ and is designed to offer scheduled wellness check-ins and communication support to help individuals stay connected with family members, caregivers, and designated emergency contacts.
Our goal is to provide peace of mind through regular check-ins and timely communication when a scheduled response is not received.
What Gracefully Connected Is
Gracefully Connected is designed to:
Provide scheduled wellness check-ins by text message, phone call, or other supported communication methods.
Help individuals remain connected with family members and caregivers.
Notify designated emergency contacts when a scheduled check-in is missed, according to the selected service plan.
Offer an additional layer of support for individuals living independently.
What Gracefully Connected Is Not
Gracefully Connected is not:
A medical alert system
A personal emergency response system (PERS)
A substitute for 911 or emergency services
Medical monitoring
A healthcare provider
A fall detection system
A guarantee of personal safety or wellbeing
If you or someone else is experiencing a medical emergency, call 911 or your local emergency services immediately.
Service Availability
Gracefully Connected relies on third-party communication providers, internet connectivity, cellular service, and user devices.
While we strive to provide reliable service, we cannot guarantee uninterrupted availability or successful delivery of every communication.
Delays or interruptions may occur due to circumstances beyond our control, including but not limited to:
Cellular outages
Internet disruptions
Power failures
Device malfunctions
Weather events
Third-party service interruptions
Emergency Contacts
When a scheduled check-in is missed, Gracefully Connected will make reasonable efforts to follow the notification process associated with the selected service plan.
Notification of emergency contacts does not guarantee immediate assistance, emergency response, or intervention.
Users are responsible for ensuring their emergency contact information remains accurate and up to date.
Privacy
Gracefully Connected respects your privacy.
Information collected through the service is used solely to operate the program, provide requested services, communicate with designated contacts when appropriate, and improve the user experience.
