Gracefully Connected™ is a service provided by Gracefully Aging Network™ and is designed to offer scheduled wellness check-ins and communication support to help individuals stay connected with family members, caregivers, and designated emergency contacts.
Our goal is to provide peace of mind through regular check-ins and timely communication when a scheduled response is not received.
What Gracefully Connected Is
Gracefully Connected is designed to:

Provide scheduled wellness check-ins through automated telephone calls and any additional communication methods expressly offered under the subscriber's selected service plan.

Help individuals remain connected with family members and caregivers.

Notify designated emergency contacts when a scheduled check-in is missed, according to the selected service plan.

Offer an additional layer of support for individuals living independently.
What Gracefully Connected Is Not
Gracefully Connected is not:

A medical alert system

A personal emergency response system (PERS)

A substitute for 911 or emergency services

Medical monitoring

A healthcare provider

A fall detection system

A guarantee of personal safety or wellbeing

Service Availability
Gracefully Connected relies on third-party communication providers, internet connectivity, cellular service, and user devices.
While we strive to provide reliable service, we cannot guarantee uninterrupted availability or successful delivery of every communication.
Delays or interruptions may occur due to circumstances beyond our control, including but not limited to:

AI-Powered Wellness Check-Ins
Gracefully Connected™ uses automated communication technology, including AI-powered telephone conversations, to conduct scheduled wellness check-ins.
These interactions are not conducted by a live human representative unless otherwise expressly stated.
Check-in conversations and related service records may be retained and reviewed by authorized Gracefully Aging Network personnel for service administration, quality assurance, follow-up, and implementation of the subscriber's selected notification procedures.
AI-powered conversations may occasionally misunderstand responses or experience technical limitations.
Gracefully Connected™ should not be relied upon to identify medical emergencies or determine whether a subscriber is safe.
Emergency Contacts
Gracefully Connected™ follows the check-in and notification procedures associated with the subscriber's selected service plan.
When a scheduled check-in is missed, Gracefully Connected will make reasonable efforts to follow the notification process associated with the selected service plan.
If a scheduled check-in is unsuccessful, the service may attempt additional contact and notify designated emergency contacts according to the established escalation process.
Notification attempts do not guarantee that an emergency contact will receive, acknowledge, or respond to a communication.
Notification of emergency contacts does not guarantee immediate assistance, emergency response, or intervention.
Gracefully Connected™ does not provide continuous monitoring, dispatch emergency responders, or guarantee that assistance will be available.
Users are responsible for ensuring their emergency contact information remains accurate and up to date.
Subscribers and their authorized representatives are responsible for maintaining accurate emergency contact information and appropriate emergency response arrangements.
Privacy
Gracefully Connected respects your privacy.
Information collected through the service is used solely to operate the program, provide requested services, communicate with designated contacts when appropriate, and improve the user experience.

Gracefully Connected™ is an active wellness check-in service. Features, communication methods, subscription options, and technology integrations may evolve as we continue improving the program.
